Welcome to Trinity Health IHA Medical Group, Primary Care – Freedom! We are a Trinity Health IHA Medical Group practice that provides care for adolescents aged 16 to adults. Our physicians and staff are dedicated to providing the highest quality medical care to our patients and the best service to their families.
