Description
Team Select Home Care serves medically complex children and adults across Howard, Miami, Grant, Tipton, Clinton, Carroll, and Cass Counties in North Central Indiana, with our office in Kokomo. Patients & Families: We offer pediatric and adult PDN, including ventilator care, trach management, and 24/7 skilled nursing, plus Personal Attendant Services. Referral Sources: We coordinate with physicians, Indiana Medicaid, and insurance for authorizations and complex case management. Nurses & Caregivers: RN, LPN, and personal care openings with flexible schedules, new grad training, and tuition reimbursement.
