Description
Team Select Home Care serves medically complex children and adults across West Central Indiana, including Tippecanoe, Montgomery, Clinton, and surrounding counties, with our office in Lafayette. Patients & Families: We offer pediatric and adult PDN, including ventilator care, trach management, and 24/7 skilled nursing, plus Personal Attendant Services. Referral Sources: We coordinate with physicians, Indiana Medicaid, and insurance for authorizations and complex case management. Nurses & Caregivers: RN, LPN, and personal care openings with flexible schedules, new grad training, and tuition reimbursement.
