Team Select Home Care provides expert in-home care for medically complex children and adults across Northeast Indiana counties, with our office in Fort Wayne. Patients & Families: We specialize in pediatric and adult private duty nursing — including ventilator care, trach management, and 24/7 skilled nursing — plus Personal Attendant Services and Indiana's Family Caregiver Program. Referral Sources: We coordinate with physicians, Indiana Medicaid, and insurance to manage authorizations and care planning for complex and overnight cases. Nurses & Caregivers: RN, LPN, and caregiver openings with flexible schedules, continuing education, new grad training, and tuition reimbursement.
